Your Child's First Visit

What to Expect
For your child’s first visit to Children’s Dental Center of Madison, we like to do a comprehensive examination. This includes looking at your child’s teeth along with the supporting structures of the gums. The dentist will also address your child’s oral hygiene, as well as clean your child’s teeth. We will look at your child’s bite and will only take X-rays if necessary. This is the optimal time to ask questions about your child’s mouth and teeth.
Preparing for Your Child’s First Visit
A common complaint of parents is that their child is afraid of going to the dentist. At Children’s Dental Center of Madison, we are experienced in making a child’s first visit to the dentist as easy and comfortable as possible.
Keep in mind that some children are more nervous or anxious in new situations or environments. That is perfectly understandable. We encourage parents to be open and honest with their children about questions or fears they have regarding their first visit to the dentist.

You may print off the forms, fill them out, and bring them with you to your appointment.
This should be filled out once per child.
This should be filled out once per household.
Online Forms
For your convenience, you can fill out all of your patient forms electronically prior to your first visit. In addition, you may use these forms to update your information every year before you come to the office.
The forms come in two parts, please complete BOTH FORMS before coming in.
Health History Form
This should be filled out once per child.
Financial/Insurance Form
This should be filled out once per household.
Make sure your appointment is scheduled with our office and we’ll see you there! No need to print your forms out. We will receive them electronically.
